Why don’t we accept Medicare and the benefits of not using Medicare?
Not many know how the Medicare System works (in Australia, overseas systems can be similar), and it’s important you are aware of all the options available to you:
Reasons for Not Accepting Medicare
Benefits of Not Using Medicare
Mental Health Care Plan remains permanently on health record
No referral required, no Medicare rebates, quicker access to therapy
Assessment and referral from and monitor Mental Health Care Plan
Individualized therapeutic approach
Waiting list to see a psychologist or psychiatrist
Able to see counsellor sooner, no waiting list
Medicare covers only $120 of therapist’s fee $300
Leaving a gap to pay $180
No Medicare rebates.
Full fee payable similar when factoring in gap fee and GP fees
- If you see a therapist under the Medicare system, you will also have a Mental Health Care Plan set up which remains on your health record permanently. This can be problematic, as once this is permanently on your record, this plan may be considered during any medical checks needed for things such as life insurance, overseas travels etc. where you may not want to share your medical information.
- You will need an assessment and referral from your GP. As such, the sessions may be limited and would require a new referral for each session pack.
- You’ll also need to factor in any fees you’ll need to pay to see your GP to set up and monitor the Mental Health Care Plan regularly.
- You may also find there is usually a waiting list to get into a therapist.
The benefits of seeing us are many without Medicare:
- No referral required and No Medicare rebates. You will likely be able to see us a lot sooner, which helps with talking through your situation more quickly.
- You may also find it is not much different in cost when you consider the gap fee to see a therapist on Medicare and also your GP’s fees.
- We can also work from a therapeutic approach that is more individual to you.
4. Example of Medicare coverage vs. out-of-pocket to visiting a counsellor:
Other Therapists
Nafs Counselling
Therapist Fee
$300
$180
Medicare Coverage
($120)
No Medicare
Gap (remainder out-of-pocket)
$180
$180
Medicare covers about $120 of the therapist’s fee and you will still need to pay the remaining fee. We find most therapists are charging about $300 per session so this leaves a gap to pay (e.g. $300-$120 = $180) which is quite similar to visiting a counsellor, but without the referral, permanent mental health record and waiting time.

For example, if appointment is at 12:00 and you don’t arrive until 12:05, that’s OK, I can still work with you, but your session ends at 1:00. Period.
And will be charged the full session fee. - After that, if you miss an appointment or cancellation occurs without 48 hours’ notice, then a 100% of your appointment will apply.

- Where you had an unavoidable sickness (unpredicted)
